About this role… We are looking for a Support Worker to join the Rapid Response Outreach Team, a Pan-London service working at night, finding and supporting people who are rough sleeping. Thames Reach’s Outreach teams operate across London and help those who are rough sleeping to find routes away from the street. The teams respond to referrals and work flexibly to reach London’s most vulnerable. Individuals are assessed to identify their support needs and support plans are created, including work to obtain identification, access health services and refer to other specialist agencies, including those with immigration issues. Thames Reach welcomes applications from people with transferable skills. You will: - Work as part of a large team that provides a nightly service to rough sleepers in London. - Plan your shift to provide a rapid response to referrals received, ensuring people sleeping rough are looked for within tight time scales. - Be the first point of contact and support for people rough sleeping across the city. - Assess the circumstances and support needs of rough sleepers you encounter and identify routes away from the streets. - Refer rough sleepers onto partner teams and into services. - Provide detailed recording of support assessments on the CHAIN database and for handover to partners. To succeed as a Support Worker, you will have: - A clean UK driving license and an ability to work nights (8pm to 4am) and a flexible shift pattern across Monday to Sunday. - The ability to work independently and as part of a team. - An enthusiastic approach to taking part in training and acquiring new skills. - Excellent communication skills to quickly engage rough sleepers that you encounter so that they accept the support you are offering. - Attention to detail with recording and the ability to represent important information to partner services in a timely manner. - The ability to work remotely and have good IT skills with experience of online recording systems and using a tablet and a smart phone.
