We work to prevent vulnerable people from becoming homeless. We help formerly homeless people get their lives back on track. About this role… We are looking for a Support Worker to join our Staying Well Team, part of the Living Well Network Alliance. Since July 2018, several organisations, including the South London and Maudsley NHS Foundation Trust and Lambeth Council, have been working together to provide adult mental health services in Lambeth. This includes the Living Well Network, which offers support in housing, employment, and community services. The Staying Well Team helps connect primary care services including GPs, and secondary mental health care. We support clients to stay well and help them transition smoothly between services. Our team provides personalised short-term and long-term support tailored to meeting each client’s needs. The successful candidate will provide varying levels of support to approximately 30 clients in the borough of Lambeth. Thames Reach welcomes applications from people with transferable skills. We will not accept applications where Artificial Intelligence (AI) has been used to assist with completing the competency questions. You will: .Conduct regular assessments of service users’ needs to tailor support effectively and ensure progress toward their goals. .Collaborate with other departments to ensure the highest quality of care for service users. .Develop SMART goals (Specific, Measurable, Achievable, Relevant, Time-bound) and assist service users in reaching these objectives. .Promote engagement and social inclusion, while fostering autonomy and independence, to enhance overall physical health outcomes. .Provide a range of interventions, both short-term and long-term, tailored to the individual needs of each service user. To succeed as the Support Worker Role, you will have: .Experience in supporting service users with mental health challenges, demonstrating empathy and understanding. .Strong prioritisation skills and the ability to manage your workload independently and efficiently. .Proficiency in Microsoft Office applications, including experience with databases to track and manage information effectively. .A proactive approach with keen attention to detail, ensuring high-quality support and documentation. .Excellent interpersonal skills to develop and maintain positive working relationships with service users and colleagues. .A genuine interest in mental health and a commitment to making a positive impact on the lives of individuals facing both short-term and long-term challenges.
