About the Role
The organisation and delivery of primary care services within East London NHS Foundation Trust (ELFT) is undergoing transformation to provide more integrated care in collaboration with internal and external partners. The portfolio of primary care services directly provided by the Trust is expanding, with a new model of service delivery that encourages greater direct participation of service users. This role involves working within directly provided primary care services to specialist homeless practices across London, contributing to the delivery of high-quality, person-centred care in this specialised setting.
Experience
Experience working within primary care or community health settings is desirable. Previous involvement with vulnerable or homeless populations is advantageous but not essential.

You will be motivated to work collaboratively within a multidisciplinary team and committed to delivering compassionate care. Strong communication skills and the ability to engage effectively with diverse service users are essential. You should be adaptable to change and keen to contribute to service development and improvement.
Qualifications
