About the Role
As a Band 5 Rehabilitation Physiotherapist, you will promote, maintain and develop a high quality, efficient and effective physiotherapy service within the Community Independence Service. You will work in a multi-disciplinary context, collaborating closely with Occupational Therapists, Nurses, Paramedics, Rehabilitation Assistants, Pharmacists and Administrators. Your role includes independently assessing the health and social care needs of patients to plan appropriate community services. Treatment is delivered in a variety of settings including patients' homes, residential and nursing homes. Travel may be required by foot, public transport, bicycle, motorcycle or car.

You will be an active member of the multi-disciplinary team, participating in clinical projects, service developments, audits and reviews as agreed with the Team Lead. Supervision of Rehabilitation Assistants and Occupational Therapy students on placement will also be part of your responsibilities. You will ensure that patients’ individual needs, cultural beliefs, dignity and privacy are respected at all times. Building and maintaining close working relationships with primary, secondary, voluntary and social care colleagues is essential to ensure patients receive the right service at the right time and in the right environment.

Experience
Experience in physiotherapy practice within a community or rehabilitation setting is desirable. Ability to work independently and as part of a multi-disciplinary team is essential.

You should have excellent communication and interpersonal skills, with the ability to build effective working relationships across a range of health and social care professionals. Flexibility and adaptability to work in various community settings and travel between locations is required.

Qualifications
You must hold a relevant degree in Physiotherapy and be registered with the Health and Care Professions Council (HCPC). Additional training or experience in community rehabilitation is advantageous but not essential.
