About the Role
The Care Coordinator will manage patient signposting and referrals across various practices, ensuring consistent communication with referrers regarding patient progress and any complications. You will proactively participate in health promotion and screening programmes, supporting personalised care requirements using decision support aids. Working closely with the Clinical Director, Strategic and Operations Director, Practice Managers, GPs, and other staff, you will deliver workstreams under the PCN contract and support patients with long-term conditions through holistic care and shared decision-making conversations.
Key responsibilities include assisting patients with queries, managing appointments, facilitating access to self-management education and peer support, and supporting patients to take control of their health and wellbeing. You will coordinate and navigate care across health and social services, promote PCN health initiatives such as immunisations and national screening programmes, and support call and recall processes.
Additionally, you will help organise multidisciplinary team (MDT) meetings, liaise with healthcare professionals and partner agencies, and support Quality and Outcomes Framework (QOF) and contractual specifications. The role involves managing patient-initiated calls, documenting and monitoring service delivery, and responding appropriately to emergency situations.
You will also bring together care and support needs into a single Personalised Care and Support Plan (PCSP), support patients in understanding and improving their health knowledge and confidence, and assist with accessing training, employment, and benefits where eligible. The role requires strong organisational skills to prioritise workload independently and collaboratively within the multidisciplinary team.
Maintaining confidentiality is paramount, as you will have access to sensitive patient and staff information, which must be handled in accordance with data protection policies and procedures.
Experience
Experience in patient coordination, health promotion, or working within primary care settings is desirable. Ability to manage multiple tasks and communicate effectively with patients and healthcare professionals is essential.
