Hours
36 hours per week, working five days including Saturdays, Sundays and Bank Holidays. The role involves a weekend and holiday working pattern with three days as part of a team and the weekend/holiday periods as the sole Social Worker on site, supported remotely by a Duty Manager.

Salary
£42,912 - £54,360 per annum, negotiable depending on experience

About the Role
As a Social Worker within the Hospital Discharge Team, you will be based at St Mary’s Hospital and work across the Boroughs of Hammersmith and Fulham, the Royal Borough of Kensington and Chelsea, and Westminster City Council. You will collaborate closely with health colleagues in an acute setting as part of a ward multi-disciplinary team. Your responsibilities will include undertaking assessments of need, developing support plans to facilitate patient discharge, conducting safeguarding investigations, and performing Mental Capacity Act assessments. You will work with carers and professionals to manage risk and support patients in maintaining their chosen lifestyle.

You will provide advice and information to colleagues and stakeholders about the social work role and services, participate in urgent response and duty systems, and contribute to service development. Maintaining your professional registration and DBS check is essential, alongside responsibility for your own continuing professional development. You will engage in appraisals, audits, evaluations, reflective practice, and team meetings.

Compliance with all council policies and procedures including health and safety, data protection, supervision, and information governance is required. You will be expected to report significant service-related events to your manager. Development opportunities include best interest and practice teacher training and Trusted Assessor training, enabling you to prescribe simple equipment and assistive technology to enhance customer independence. An inter-disciplinary and partnership approach is fundamental to achieving the best outcomes.

Requirements

Experience
Post-qualifying experience in a relevant Local Authority or Health setting is required. Experience working in an acute setting and within a multi-disciplinary team is essential. You should have experience of assessment work, safeguarding investigations, and Mental Capacity Act assessments, particularly with older or physically disabled people in a Social Services Duty System.

About you
You will have well-developed knowledge of relevant legislation and a strong understanding of personalisation, independence, and choice principles. Excellent written and verbal communication skills are essential for producing high-quality reports and interacting with carers and professionals. You should be able to work independently, use initiative, prioritise workloads, and meet deadlines. Confident computer skills, particularly with Microsoft Office, are required. You will be proactive in negotiating with other agencies and managing issues, with strong case recording, assessment, risk management, and budget understanding. The ability to communicate professionally in a language other than English is desirable.

Qualifications
You must hold a CQSW/Diploma in Social Work, a Degree in Social Work or its equivalent, and be registered with Social Work England.
