About the Role
As a Community IV Nurse, you will deliver safe, high-quality, patient-centred care in patients' homes and community settings. Your responsibilities will include administering prescribed Parenteral Nutrition (PN), IV therapies, and clinical interventions in line with SOPs and professional standards. You will assess patients, identify changes in condition, and escalate clinical concerns appropriately. Supporting treatment initiation, ongoing care, and transitions from hospital to home will be key aspects of your role. You will train, educate, and empower patients, families, and carers to safely manage treatment and build independence. Providing clinical advice and support while building strong relationships with patients and healthcare professionals is essential. You will work collaboratively with NHS colleagues, Senior Homecare Nurses, Clinical Nurse Managers, and wider teams to ensure continuity of care. Maintaining accurate clinical records and working in line with Clinical Governance, NMC standards, and Inizio policies is required. You will also support patient safety, quality improvement, service development, and achievement of programme KPIs. Maintaining clinical competence through training, CPD, reflective practice, and evidence-based care is expected.

Experience
Experience in community nursing or IV therapy administration is preferred. Ability to deliver patient-centred care in home and community settings.

Qualifications
