About the Role

The Crescent Day Service supports adults with learning disabilities and complex needs by providing a range of meaningful activities in partnership with carers, health professionals, and multidisciplinary colleagues. We are seeking highly motivated individuals who are compassionate, effective communicators, and able to motivate service users to achieve quality outcomes.
As a Day Services Facilitator, you will work closely with a small number of service users as their keyworker, supporting personal care needs while promoting independence, choice, dignity, health, and wellbeing. You will contribute to the development and promotion of the Day Services, working as part of a team committed to Equality, Diversity, and Inclusion (EDI). Regular supervision and ongoing training will be provided to support your career development in Adult Social Care.

Experience
Some experience in a day service or social care setting is required, or a demonstrated good understanding of supporting individuals with profound and multiple learning disabilities or Dementia to achieve the best outcomes.

You are compassionate, motivated, and able to communicate effectively. You have good literacy, numeracy, and IT skills. You are confident working both independently and as part of a team. A can-do attitude and commitment to promoting Equality, Diversity, and Inclusion are essential.
Qualifications
No specific qualifications are mandated, but a commitment to ongoing training and development in Adult Social Care is expected. You will be required to undergo an Enhanced Disclosure and Barring Service (DBS) check and a Cifas check as part of the recruitment process.
