About the Role
As an INT Caseworker, you will play a vital role in supporting patients through holistic assessments and coordinated care within the Integrated Neighbourhood Team. You will analyse population health data to identify high-priority patients and manage the INT caseload efficiently. Working closely with multidisciplinary teams, you will coordinate patient support plans and facilitate group consultations for long-term conditions such as Type 2 Diabetes, Atrial Fibrillation, Chronic Kidney Disease, and Hypertension.
Your responsibilities will include conducting face-to-face holistic assessments, performing basic health checks such as blood pressure, weight, height, pulse, and blood tests (training provided). You will work with patients to set SMART goals that promote independence and well-being, encouraging self-management wherever possible.
In addition, you will provide effective signposting to community and support services, act as a liaison between primary, secondary, social care, mental health, and voluntary sectors, and ensure patients are connected to appropriate resources. You will participate in INT and MDT meetings to advocate for patient needs and maintain accurate records of interventions and progress.
Utilising digital tools and IT systems such as EMIS Web, Microsoft Excel, AccuRx, Ardens Manager, and Microsoft Outlook, you will manage patient data and communications efficiently. You will also support digital inclusion by assisting patients in accessing online services.
Building strong partnerships with local voluntary and community organisations will be key to enhancing support pathways and improving service delivery based on patient and partner feedback.
Experience
Experience in population health management, patient assessment, or care coordination is desirable. Familiarity with IT systems such as EMIS Web, Microsoft Excel, and AccuRx is beneficial.

Flexibility to work outside core hours when required is essential. You are confident in using digital tools and committed to promoting patient independence and well-being.
Qualifications
Relevant health or social care qualifications are advantageous but not mandatory. Training will be provided for specific clinical tasks such as blood testing.
