About the Role
Following an assessment, you will provide practical and recovery-focused support to service users and carers as outlined in their care plans. Working closely with care coordinators or lead clinicians, you will deliver clearly defined recovery-focused interventions in line with individual care packages. You will attend and actively participate in team meetings, care plan review meetings, and team training as appropriate. Your role will involve positively promoting and supporting independent living for service users in the community by maintaining extensive knowledge of and links with community resources, actively supporting service users to access them. Developing a professional rapport with service users based on support and collaboration within appropriate boundaries is essential. You will provide regular practical support to service users and carers to maintain dignity and independence, enabling service users to acquire daily living skills such as shopping and cooking. Additionally, you will assist service users in accessing resources including benefits and welfare rights, health promotion information, and physical health services while monitoring their health needs in the community. You will help identify early signs of relapse by monitoring clients’ progress, level of functioning, and mental state, updating care coordinators or other involved staff accordingly.

Requirements

Experience
Experience working in a recovery-focused mental health environment is desirable. Ability to work collaboratively with multidisciplinary teams and service users.

Committed to supporting service users to live independently and with dignity. Able to develop professional relationships within appropriate boundaries. Knowledgeable about community resources and motivated to help service users access these. Strong communication and observational skills to monitor clients’ progress and identify early signs of relapse.

Qualifications
Relevant qualifications or experience in mental health or social care are advantageous but not explicitly stated. A willingness to learn and develop professionally within the role is essential.
