About the Role
The Mental Health and Wellbeing Practitioner will work within the Older Adults Community Mental Health Team, supporting older adults experiencing mental health difficulties, including those living with cognitive impairment and/or frailty. Working as part of a multidisciplinary team, the post holder will contribute to assessment, care planning, risk management and the delivery of person-centred, recovery-focused care. The role involves working collaboratively with service users, families and carers to develop strengths-based care plans, coordinate interventions and support shared decision-making. The practitioner will provide evidence-based, psychologically informed interventions under the supervision of a Clinical Psychologist, including behavioural activation, problem-solving, emotional wellbeing support, sleep management, confidence building, medication support and relapse prevention planning. Key responsibilities include carrying out risk assessments, monitoring service users’ progress, maintaining accurate clinical records, contributing to multidisciplinary discussions and liaising effectively with healthcare professionals, social care and third-sector organisations to ensure continuity of care. The successful candidate will demonstrate compassion, empathy and excellent communication skills when working with individuals experiencing distress or complex mental health needs. The role offers an excellent opportunity to develop specialist skills within older adult mental health while contributing to high-quality care and positive outcomes for service users and their carers. The post holder will actively participate in clinical and case management supervision, quality improvement activities and ongoing professional development, maintaining high standards of clinical practice and professional conduct at all times.
Experience
Experience working with older adults experiencing mental health difficulties is desirable. Experience in delivering psychologically informed interventions and working within a multidisciplinary team is advantageous.

Qualifications
Relevant qualifications in mental health, psychology, social work, nursing or a related field are expected. Experience or training in delivering evidence-based psychological interventions under supervision is desirable.
