About the Role
Would you like to work somewhere where you get to make a real difference every day? As Senior Practitioner, you will work alongside your Support Manager to provide an excellent support service for customers in one location in Hackney, London. You will lead, develop, monitor, and evaluate support practice in line with Advance policies and procedures, best practice, contractual and regulatory standards. You will be passionate about ensuring the service delivered is person-centred and supports people to develop their confidence, skills, and independence in their own home and local community.
The role includes overseeing the support given by support workers and providing practice leadership for the team by:
- Supporting and observing support workers to ensure best practice
- Coaching team members to manage behaviours that challenge, acting as a role model and encouraging a reflective culture
- Working effectively across a range of teams including peer groups, Multi-Disciplinary, and Specialist Health Teams
- Supporting the review and update of support plans and related customer documents
- Collaborating with the Support Manager and Support Workers to develop and implement plans that promote communication and support decision-making
- Taking an active role in training, coaching, and developing staff on Mental Health and person-centred delivery, including organising community of practice groups, training events, and workshops
- Being alert and proactive in safeguarding the health, safety, and welfare of customers, yourself, and colleagues
- Attending meetings to discuss customer progress and plan future transitions
Experience
Experience working with people with learning disabilities or Mental Health support is desirable but not essential. Training will be provided to the right applicant.
