About the Role
Are you motivated by delivering outstanding practice within a supportive, forward-thinking Adult Social Care service? Join the Richmond & Barnes Social Care Team, a friendly and well-established multidisciplinary team co-located with District Nurses and Community Matrons. This role offers the opportunity to shape and strengthen Adult Social Care across Richmond and Wandsworth.
You will work with adults aged 18+ with physical disabilities and older adults with complex needs, using personalised and strengths-based approaches to support their independence and wellbeing. The team offers manageable caseloads, flexible working arrangements, and a supportive culture.
Based at our East Sheen office, just 12 minutes from Clapham Junction with onsite parking, you will be well placed to collaborate with local GP practices, the library, Sheen Day Centre, and the Alzheimer’s Carers Café, facilitating effective partnership working.
Key Responsibilities
- Carry out holistic assessments of individuals and carers, identifying needs, risks, and opportunities for independence.
- Work collaboratively with residents, families, and partners using a strengths-based, person-centred approach.
- Apply social work principles, professional curiosity, and anti-discriminatory practice throughout your work.
- Support residents to maximise independence, promote wellbeing, and prevent harm.

Experience
- Post-qualification experience in Adult Social Care.
- Strong knowledge of relevant legislation and local authority duties, particularly around safeguarding.

Qualifications
- Recognised Social Work qualification.
