About the Role
Join Thames Reach as a Support Worker at Martha Jones House, a transitional hostel where a 24-hour staff team supports people experiencing homelessness. You will help clients move in, assess their needs, and develop personalised support plans to assist them in moving on to suitable accommodation. Your role will involve supporting clients with health, substance misuse, criminal justice, and life skills needs, ensuring a safe and supportive environment that reflects our ethos. You will contribute to effective day-to-day service delivery, conduct risk and needs assessments, liaise with external agencies and councils, and work towards increasing client engagement and successful move-on outcomes within set timeframes. Thames Reach is committed to a culture of high performance and continual improvement, and you will play a key role in fostering this within the team.

Experience
