Sutton Adult Social Care Locality Teams support adults aged 18 and over with care and support needs to live independently, safely, and with dignity within their communities. The teams undertake Care Act assessments, reviews, safeguarding enquiries, mental capacity assessments, support planning, and work in partnership with health, housing, voluntary sector, and community services.
As a Social Worker in the Locality Team, you will adopt a strengths-based and person-centred approach, promoting independence, wellbeing, choice, and control. You will work with a diverse range of individuals including older adults, people with physical and learning disabilities, mental health needs, and those experiencing self-neglect or safeguarding concerns.

Experience
- Experience working with adults in a statutory social care setting, including assessments, reviews, and safeguarding work.
- Knowledge of relevant legislation including the Care Act 2014, Mental Capacity Act 2005, and safeguarding responsibilities.
- Experience managing complex cases involving risk and professional judgement.
- Desirable: Experience working within a Locality Team or integrated health and social care setting.
- Desirable: Experience with self-neglect, hospital discharge pathways, Continuing Healthcare, or complex safeguarding enquiries.
- Desirable: Practice Educator qualification or interest in supporting student social workers and newly qualified staff.
