Adult Social Care is undergoing significant transformation to meet rising demand and improve service delivery. The Hospital Pathway and Reablement (START) Service works closely with NHS colleagues to facilitate safe and timely discharge, focusing on enabling people to return to their own homes and regain independence through reablement and other services.

As a Social Worker within the Hospital Pathways Team, you will conduct assessments, manage complex cases, and deliver high standards of care. You will develop outstanding care plans that prioritise the needs and voices of service users, collaborating with professionals, families, carers, and external agencies to achieve the best outcomes.

Experience
- Qualified Social Worker with current registration with Social Work England
- Minimum 2 years post-qualifying experience in adult services, including assessment and support of individuals with diverse needs
- Successful completion of ASYE (Assessed and Supported Year in Employment)
- Experience managing complex cases and working collaboratively with multi-disciplinary teams

Qualifications
- Current registration with Social Work England
- Sound knowledge of relevant legislation including the Care Act 2014 and Mental Capacity Act 2005
- Strong skills in assessment, outcome-focused care planning, and risk management
- Ability to communicate complex social issues clearly to patients and professionals
- Ability to plan and coordinate social care plans and aspects of patient discharge
