The borough is committed to supporting residents and vulnerable groups to maintain independence and balance support with independent living.

Adult Social Care is undergoing a significant transformation to meet rising demand, with new leadership driving innovative changes to improve care and create positive working environments. As a Social Worker within the Hospital Pathways Team, you will conduct assessments, manage complex cases, and provide high standards of care. The team is known for crafting outstanding care plans that prioritise service users’ needs and voices, integrating input from professionals, families, carers, and external agencies to achieve the best outcomes.

This role offers a Welcome Bonus of £1,000 paid in the first month’s salary or a Retention Bonus of £1,000 for every complete year in service (subject to pay scale). You will be supported to grow your career in a dynamic, diverse environment dedicated to excellence in adult social care.
