Adult Social Care is undergoing a significant transformation to meet rising demand, with new leadership driving innovative changes to improve care and create positive working environments. The Hospital Pathway and Reablement (START) Service works closely with NHS colleagues to facilitate safe and timely discharge, prioritising enabling people to return to their own homes and regain independence through reablement and other services.
As a Social Worker within the Hospital Pathways Team, you will conduct assessments, manage complex cases, and provide high standards of care. The team is known for crafting outstanding care plans that prioritise service users’ needs and voices, integrating input from professionals, families, carers, and external agencies to achieve the best outcomes.
This role offers a Welcome Bonus of £1,000 paid in the first month’s salary or a Retention Bonus of £1,000 for every complete year in service (subject to pay scale). You will be supported to grow your career in a dynamic, diverse environment dedicated to excellence in adult social care.
Experience
- Minimum 2 years post-qualifying experience working in adult services
- Experience assessing and supporting individuals with diverse needs
- Successful completion of the Assessed and Supported Year in Employment (ASYE)

Qualifications
- Qualified Social Worker with current registration with Social Work England
- Eligibility to work in the UK
- Sound knowledge of relevant legislation including the Care Act 2014 and Mental Capacity Act 2005
- Strong skills in assessment, outcome-focused care planning, and risk management
- Ability to communicate complex social issues clearly to patients and professionals
- Ability to plan and coordinate social care plans and aspects of patient discharge
