About the Role
We are seeking an experienced and highly motivated Advanced Social Work Practitioner to join our Preparation for Adulthood Team. This key role supports young people with complex needs as they transition from Children's Services into Adult Social Care. The successful candidate will manage a complex and demanding caseload autonomously, providing high-quality social work practice and delivering positive outcomes for young people and their families. You will also support the development of less experienced staff through supervision and line management responsibilities. Key responsibilities include managing complex cases, undertaking strengths-based assessments, developing person-centred support plans, ensuring statutory duties are met within required timescales, working collaboratively with partner agencies, and leading on complex decision-making and service development.

Requirements

Experience
- Extensive post-qualifying social work experience, particularly within Transitions and Preparation for Adulthood services
- Strong knowledge of the Care Act 2014, Mental Capacity Act 2005, and relevant statutory guidance
- Experience with Transitions from Children's to Adult Services, Deprivation of Liberty Safeguards (DoLS), Community DoLS, SEND legislation, and Education, Health and Care Plans (EHCPs)
- Skilled in completing NHS Continuing Healthcare (CHC) Checklists and supporting assessments
- Experience working in partnership with Integrated Care Board (ICB) and health colleagues
- Proven ability to work with young people with learning disabilities, autism, physical disabilities, and/or complex health needs
- Ability to prioritise competing demands, manage workloads effectively, and meet deadlines consistently
- Excellent assessment, analytical, communication, and report-writing skills

You are a highly motivated and autonomous social work professional with a commitment to delivering positive outcomes for young people and their families. You demonstrate strong leadership skills, including supervision and line management of staff. You work collaboratively with partner agencies and contribute to service development. You are adaptable, organised, and able to manage complex and high-risk caseloads with sound professional judgement.

Qualifications
- Relevant social work qualification with professional registration
- Evidence of continuous professional development relevant to the role
