We are looking for an experienced and motivated Advanced Social Worker to join our LIFE (Living Independently for Everyone) Team. This is a fast-paced role focused on hospital discharge, working within the Discharge to Assess (D2A) model. You will support individuals to return home safely, delivering strength-based assessments that promote independence and recovery.

Key responsibilities include:
- Leading and delivering high-quality strength-based assessments in the community
- Supporting hospital discharge pathways through short-term, outcome-focused interventions
- Providing supervision, mentoring, and guidance to Social Workers and Health & Wellbeing Assessors
- Participating in the Duty Senior rota, allocating work and offering professional advice
- Working within a multi-disciplinary, integrated team to deliver person-centred care
- Contributing to service development and continuous improvement

Requirements

Experience
A qualified Social Worker with post-qualifying experience. Experienced in adult social care practice, ideally including discharge or reablement. Confident in supervision and supporting staff development.

About you
Skilled in strength-based practice and person-centred approaches. Able to work flexibly in a fast-paced and evolving environment. A collaborative team player with professionalism, creativity, and drive.

Qualifications
Qualified Social Worker status with relevant post-qualifying experience.
