About the Role
This role involves providing expert guidance and support to individuals living with dementia and their families. As a Bilingual Dementia Care Navigator/Advisor, you will work closely with multidisciplinary teams to ensure patients receive timely and appropriate care. The position requires adaptability as duties may evolve to meet the changing needs of the service. You will be a key point of contact, helping to navigate complex care pathways and improve patient outcomes through effective communication and culturally sensitive support.
