About the Role
Outward has been providing high-quality support and care services to vulnerable people across London for 50 years. We specialise in person-centred support for adults with learning disabilities, autism, mental health needs, and complex needs. As a Community Outreach Officer, you will join a dedicated team delivering community-based support designed to help adults maintain or gain independence. Your role will involve supporting people to learn new skills, find social activities, manage housing matters, handle money management, benefit applications, and maintain a healthy lifestyle.
This position offers a unique opportunity to combine direct support with service coordination and professional networking. You will provide high-quality support while managing day-to-day service delivery elements such as support planning, key working, and rota session times. Working closely with the Deputy or Team Manager, you will help ensure services are person-centred, well-led, and aligned with Outward’s core values of engage, enable, and empower.

Experience
The role involves a range of support activities including:
- Claiming benefits: assisting with forms, advice, and contact with DWP services
- Budgeting: preparing and monitoring budget plans
- Employment support: encouraging job applications, CV and form assistance
- Encouraging participation in meaningful activities
- Identifying and acting on safeguarding issues
- Supporting tenancy at risk situations including legal advice and court support
- Promoting health through diet, exercise, and liaising with health services
- Attending appointments and providing detailed feedback
- Supporting household chores, cooking, and shopping with the aim of encouraging self-reliance
- Facilitating access to community resources and local services
Note: This role does not typically include hands-on personal care, regular medication supervision, holding customer finances, formal counselling, or cooking/cleaning for the customer beyond limited support.
About you
- Good understanding of safeguarding and risk management in care/support settings
- Knowledge of health and safety in supported housing or care environments
- Understanding of confidentiality, GDPR, and data protection principles
- Access to a smartphone capable of receiving SMS and installing authentication apps
- Right to Work in the UK (Outward does not provide visa sponsorship)
Qualifications
- NVQ Level 2 or 3 in Health and Social Care (or equivalent) is desirable
- Additional training in Positive Behaviour Support (PBS), safeguarding, or housing-related legislation is advantageous
