About the Role
As a Community Physiotherapist, you will work closely with patients and their carers to promote self-management strategies. You will represent the service at multi-disciplinary team meetings, contributing to discussions on patient care, progress, discharge planning, and ongoing rehabilitation needs. You will maintain accurate treatment records that meet legal, statistical, and professional standards (HPC/CSP/Trust/departmental). Comprehensive progress and discharge reports will be provided to GPs, consultants, patients, and other members of the multi-disciplinary team. Managing clinical risk within your patient caseload is essential. Flexibility to provide cover in other clinical areas during staff shortages is required.
Experience
Experience working in physiotherapy or a related clinical setting is expected to effectively manage patient care and participate in multidisciplinary teams.
About you
You are a dedicated healthcare professional with strong communication skills, able to work collaboratively within a multidisciplinary team. You demonstrate clinical responsibility and flexibility to adapt to varying clinical demands and cover other areas when needed.
Qualifications
You must hold relevant physiotherapy qualifications and be registered with the Health and Care Professions Council (HCPC). Membership of the Chartered Society of Physiotherapy (CSP) is desirable.
