About the Role
We are seeking an experienced Community Support Worker or a Qualified Social Worker to join our Social Work and Community Support team. This rewarding role offers the opportunity to make a real difference to the lives of older people, adults, carers and families within the Jewish community. Depending on your qualifications, professional registration and experience, you will be appointed as either a Community Support Worker or a Qualified Social Worker.

Working as part of a friendly, multidisciplinary team, you will provide advice, assessment, advocacy and practical support that enables people to remain independent, connected and living well within their communities.

Jewish Care's Social Work and Community Support Service provides personalised support across London, Hertfordshire, Essex and the South East. Our service helps people navigate challenges related to ageing, disability, dementia, caring responsibilities, social isolation and changing life circumstances. Through assessment, advice, advocacy and community-based support, we empower people to maintain their independence and wellbeing whilst working closely with statutory, voluntary and community partners.

Whether appointed as a Community Support Worker or Qualified Social Worker, your responsibilities will include managing a varied caseload of adults and carers, undertaking holistic assessments to identify individual strengths, needs and risks, providing information, advice, advocacy and practical support, and developing person-centred support plans that promote independence and wellbeing. You will work collaboratively with colleagues and external professionals to achieve the best outcomes, support carers and families through changing circumstances, contribute to community initiatives, support groups and service development projects, maintain accurate case records using our CRM system, and promote safeguarding, dignity, choice and inclusion.

Qualified Social Workers will also undertake more complex assessments, apply professional social work judgement and lead on safeguarding, risk management and care planning where appropriate.
