About the Role
The post holder will chair review/CPA meetings and offer crisis appointments to patients identified as being in the red zone. Responsibilities include providing home visits and telephone advice to general practitioners and other professionals involved in the care of service users on the caseload. The consultant will run four formal outpatient clinics per week, with 4-5 patients booked per clinic. Alongside other team medics, the consultant will screen new referrals to ensure prompt assessment by the team. This process follows a framework that includes offering an Initial Family Meeting (IFM) where goals, objectives, and completion of a DIALOG care-plan are agreed.
