About the Role
Are you passionate about providing strength-based social care that transforms the lives of adults with learning disabilities? Do you want to make a positive difference in the lives of adults with disabilities? If yes, we have the perfect role for you! We are seeking dedicated and enthusiastic Social Workers to join our Learning Disabilities team, delivering vital services to adults with learning disabilities and other complex needs.

In this role, you will undertake strength-based social care assessments in line with the Care Act, produce outcome-focused support plans tailored to individual needs, and deliver targeted interventions that safeguard and empower adults with learning disabilities and complex needs. You will promote independence, positive outcomes, and the effective use of assistive technology. Working collaboratively with colleagues across Council departments, NHS Southwark, and other key partners is essential.

The Learning Disability (25+) Service sits within Southwark’s All Age Disability pathway, providing integrated social care services from 0–25 and beyond. Our team offers a comprehensive pathway for adults with learning disabilities and eligible social care needs. We have two roles available in our adult duty team: one permanent position and one fixed-term position for 10 months. The adult duty function acts as the first point of contact for adults, coordinating initial responses and ensuring cases are appropriately assessed, allocated, and progressed.

We are a values-led, inclusive, and supportive team committed to professional development, reflective supervision, and co-production with service users and carers. Our culture values diversity, collaboration, and innovation, ensuring services are modern, efficient, and responsive to community needs.

An enhanced DBS check is required for this role.
