About the Role
The Mental Health Pathway Manager role is part of the Living Well Network Alliance (LWNA), a unique partnership delivering integrated Mental Health services with key partners including South London and Maudsley NHS Foundation Trust, Certitude, Thames Reach and South London ICB. This role offers a rewarding opportunity to coordinate and facilitate service user moves into, across, and out of mental health pathways, working closely with Providers, Mental Health operational health and social care staff, Housing, and Commissioning teams.
The post holder will manage flow into and out of the pathway, including identifying and managing voids, tracking service user progress towards move-on, and addressing barriers and provider issues by finding collaborative solutions. A strong focus on information management is essential, including tracking and reporting all pathway activity to support strategic decision-making.

Experience
Experience in social work or mental health adult social care services is highly desirable. Proven ability to coordinate complex pathways and work collaboratively with multiple stakeholders is essential.

Strong organisational and communication skills are required, along with the ability to manage competing priorities and maintain accurate information management for reporting purposes. Qualifications
Relevant social work qualifications or equivalent experience in mental health adult social care are required. Candidates should be registered or eligible for registration with the appropriate professional body.
