About the Role
This post is part of the Health Check Liaison Team, which operates across the Trust's community services. The team provides health checks and support to patients with serious mental illness under the care of South London and Maudsley NHS Foundation Trust. The role involves recording comprehensive information in the electronic patient record, producing high quality letters to share information with primary care, generating regular progress reports, and setting up processes to manage workload efficiently. Key physical health checks include BMI, blood pressure and pulse, lifestyle factors such as diet and exercise, smoking status, substance misuse (drugs and alcohol), cardio-metabolic risk assessment using tools like Q-Risk, blood lipids including cholesterol, and blood sugar (HbA1c). The role requires flexibility to work across all four boroughs served by the Trust. The team works in an agile way, deploying staff temporarily to teams with high need and includes home visiting with equipment. The position is primarily patient-facing within community team bases and requires adaptability regarding working location.

Experience
Experience working within community or mental health settings is desirable to effectively support patients with serious mental illness.

You will be flexible and responsive to changing work locations and team needs, able to work independently and as part of a multidisciplinary team. Strong communication skills are essential for producing clear correspondence and reports. You should be motivated to support physical health monitoring and patient care within community settings.

Qualifications
Relevant nursing associate qualifications or equivalent healthcare experience are required to undertake this role within the NHS framework.
