About the Role
Join the London Borough of Sutton’s Adult Social Care team as a Social Worker and receive a Welcome Bonus of £1000 paid in your first month's salary and a Retention Bonus of £1000 for every complete year in service (subject to payscale). Sutton is undergoing a significant transformation to meet rising demand and improve care for adults, supported by new leadership and innovative service delivery models. You will work within one of four Locality teams aligned to Primary Care Network footprints (Wallington, Carshalton, Sutton & Cheam), supporting older people, those with physical and sensory disabilities, and individuals with mental health needs.

Your key responsibilities will include undertaking Care Act 2014 assessments and reviews, managing safeguarding enquiries, conducting Mental Capacity assessments, participating in duty rota, DSTS and CHC processes, and court-related work. You will be part of a multidisciplinary team delivering person-centred care plans that prioritise service user needs and integrate input from families, carers, and external agencies.

Experience
- At least 2 years post ASYE working experience as a UK registered social worker
- Experience working in a multidisciplinary team setting
- Experience managing complex mental health and physical health cases
- Proven ability to deliver social work support to service users and their families
- Experience in planning and coordinating social care plans and patient discharge
