About the Role
Join Adult Social Care as a Team Manager and receive a generous Welcome Bonus of £1000, paid in your first month's salary. This exciting opportunity involves leading and managing the Hospital Pathway Team based at St Helier Hospital within the Transfer of Care Hub (TOCH). You will provide clear direction and support to ensure effective and timely discharges from hospital, collaborating closely with healthcare professionals, families, and external agencies to develop comprehensive discharge plans and deliver person-centred care. Your leadership will help improve outcomes, reduce length of stay, and promote seamless transitions to community care. This role is initially offered on a 12-month fixed term contract with the possibility of becoming permanent. Reporting directly to the Head of Service and working alongside another Team Manager, you will foster a positive and inclusive team culture that encourages continuous learning and professional development.

About the Service and Team
The Hospital Pathway and Reablement (START) Service facilitates safe and timely discharges for residents transitioning from hospital to home. The multidisciplinary team works closely with NHS colleagues to provide seamless, holistic care tailored to individual needs. The service empowers residents to regain independence and confidence through personalised rehabilitation care plans, supports families and carers with knowledge and resources, and aims to prevent hospital readmissions while promoting overall wellbeing within the community.

Experience
- Extensive post-qualification social work practice experience, ideally within a hospital discharge setting
- Strong leadership and management skills with experience managing multidisciplinary social care teams
- Significant knowledge of relevant legislation including the Care Act 2014 and Mental Capacity Act 2005
- Proven ability to collaborate effectively with diverse stakeholders and community partners to achieve shared objectives
- Ability to work onsite 5 days a week at St Helier Hospital
