Situated in Zone 5, Sutton is less than 30 minutes by train from central London, and south and south west London are easily accessible by public transport. There are 10 train stations which serve key neighbourhoods within the borough providing direct connections to London Victoria, London Bridge, London Blackfriars and St Pancras International. Sutton also has direct rail connections with Epsom, Dorking, Guildford and Horsham, as well as Wimbledon for an interchange with the London Underground and West Croydon for the London Overground. There's also an extensive road network which provides close and easy access to the M25 and M23. About the Service and Team : Adult Social Care Mental Health Team Within our boroughwide specialist service, there are three social work teams - The Transition Team, The Learning Disability Team, and the Mental Health Team. The Mental Health Team's purpose is to coordinate and fulfill the Council's statutory social care responsibilities for individuals with complex mental health needs. We are a small but growing service, consisting of a welcoming management team (Team Manager and two Assistant Team Managers), Approved Mental Health Professionals, Social Workers and Enabling Independence Workers. We work closely with our health colleagues in order to complete reviews under S117 as well as facilitating timely discharges from psychiatric wards for Sutton residents. Whilst some Social Workers focus on this work, others focus on Care Act assessments and reviews for those in the community as well as completing Safeguarding enquiries, and supporting with our duty service. As part of the transformation of services within the London Borough of Sutton's Adult Social Care, we are recruiting Qualified Social Workers within the Mental Health Team. Reporting to the Management Team (Team Manager and Assistant Team Managers), you'll play a pivotal role in delivering top-quality, responsive social care and support to the residents of Sutton experiencing mental health needs. You will be responsible for meeting the statutory responsibilities under the Care Act, and other applicable policy and legislation. You will undertake assessments and reviews of need and associated support plans, safeguarding duties, S117 reviews, hospital discharge work for those in psychiatric hospitals as well as supporting a duty service. This role presents an excellent opportunity for experienced and qualified Social Workers who are confident in their ability to work with complex cases and can demonstrate outstanding communication skills with a compassionate manner, capable of effectively manage high levels of emotional distress. In addition, we would expect all suitable candidates to met the following competences: Must be registered with Social Work England and hold a degree in Social Care or similar. Direct experience of working with people experiencing mental health needs, with detailed knowledge of key legislation including the Mental Health Act, the Care Act 2014, the Mental Capacity Act, and the Human Rights Act. Have experience of working in a multidisciplinary team setting. Ability to communicate complex social issues clearly to both patients and professionals. Ability to plan and coordinate social care plans and aspects of patient discharge.
