About the Role
Join Adult Social Care as a Social Worker within the Hospital Pathways Team and receive a Welcome Bonus of £1000 paid in your first month's salary or a Retention Bonus of £1000 paid for every complete year in service (subject to pay scale within the grade). Experience
- Qualified Social Worker with current registration with Social Work England and eligibility to work in the UK.
- Minimum 2 years post-qualifying experience working in adult services, assessing and supporting individuals with diverse needs.
- Successful completion of the Assessed and Supported Year in Employment (ASYE).
- Experience in managing complex cases and delivering person-centred care.
- Sound knowledge of relevant legislation including the Care Act 2014 and Mental Capacity Act 2005.
- Strong assessment, outcome-focused care planning, and risk management skills.
- Ability to communicate complex social issues clearly to patients and professionals.
- Ability to plan and coordinate social care plans and aspects of patient discharge.
