About the Role
Join Adult Social Care as a Social Worker within the Hospital Pathways Team and receive a Welcome Bonus of £1000 paid in your first month's salary or a Retention Bonus of £1000 paid for every complete year in service (subject to pay scale within the grade). This role offers an exciting opportunity to empower adults to live as independently as possible by conducting assessments, managing complex cases, and providing high standards of care. You will work closely with NHS colleagues to facilitate safe, timely discharge and enable people to return to their own homes using reablement and other services to regain independence. Experience
- Qualified Social Worker with current registration with Social Work England and eligibility to work in the UK.
- Minimum 2 years post-qualifying experience working in adult services, assessing and supporting individuals with diverse needs.
- Successful completion of the Assessed and Supported Year in Employment (ASYE).
- Experience in managing complex cases and delivering person-centred care.
- Sound knowledge of relevant legislation including the Care Act 2014 and Mental Capacity Act 2005.
- Strong assessment, outcome-focused care planning, and risk management skills.
- Ability to communicate complex social issues clearly to patients and professionals.
- Ability to plan and coordinate social care plans and aspects of patient discharge.

Qualifications
