Working collaboratively within a multidisciplinary team, you will contribute to assessments, care planning, and risk management. The role involves delivering individual and group-based interventions, including weight management and nutritional support programmes. You will identify and manage risks related to malnutrition, obesity, and complex physical health conditions, supporting individuals, families, and carers to access appropriate health services and make informed nutritional choices. Additionally, you will develop and deliver accessible nutritional information and resources tailored to the needs of people with learning disabilities, provide training and advice to MDT colleagues, carers, and support staff, maintain accurate clinical records, and contribute to service data collection and performance monitoring. Participation in clinical supervision, service development, audit, and quality improvement initiatives is also expected.
