About the Service and Team
The Community Learning Disability Team is a friendly, motivated, and dedicated group of Social Care Staff. The team undertakes care management and safeguarding responsibilities in line with the Care Act 2014, Mental Capacity Act 2005, and other relevant adult social care legislation and guidance. They work closely with a multi-disciplinary health team including Clinical Psychologists, Speech and Language Therapists, Community Nurses, Occupational Therapists, Physiotherapists, Counsellors, and Behaviour Analysts to support adults with learning disabilities and associated complex needs. The team also provides a transition service working with Children Social Care and Special Education Needs to ensure a smooth transfer to adult services in line with the Children and Families Act 2014. Support is provided in a person-centred way to assess needs and create effective outcome-focused support plans.

About the Role and Our Ideal Candidate
An opportunity has arisen for a dedicated Support Coordinator to join the Learning Disability Team. The post holder will contribute to delivering the team’s objectives by providing effective case management. We are seeking an enthusiastic and motivated individual who understands the challenges faced by adults with learning disabilities and their families. This role offers the chance to be part of a committed and supportive team and to work in a varied and stimulating environment. Responsibilities include undertaking Care Act assessments, support planning, care plan reviews, safeguarding enquiries, multi-disciplinary working, and participating in the team’s duty system.

Requirements

Experience
Experience working with adults with learning disabilities and an understanding of the challenges they and their families face is desirable.

About you
Enthusiastic, motivated, and committed to supporting adults with learning disabilities to achieve the best possible outcomes. Able to work effectively within a multidisciplinary team and manage casework responsibly. A person-centred approach with strong communication and organisational skills is essential.

Qualifications
Relevant social care qualifications or experience in adult social care are preferred but not explicitly stated. A willingness to develop knowledge and skills through further training is expected.
