About the Role
We are seeking an Adult Social Worker to join a Community Mental Health Team supporting vulnerable adults with mental health difficulties. The role involves conducting face-to-face visits within the community, implementing care plans, and carrying out assessments. This position offers a hybrid working lifestyle with regular supervision and continuous support throughout the post. The team works closely with adults experiencing dementia, mental health challenges, older people, and physical disabilities, providing strength-based, creative solutions to meet their needs.

Experience
Minimum of 1 year post ASYE experience is required. Experience working with dementia, mental health, older people, and physical disabilities is desirable.

About you
You will be confident in carrying out adult social care assessments using strength-based approaches to identify creative solutions. You should be motivated to work within a supportive team environment and committed to delivering high-quality care.

Qualifications
Social Work Degree, DipSW, or CQSW qualification is essential.
