About the Role
A North West London local authority is seeking a Qualified Social Worker to join their Adults services within a Hospital Discharge Team. You will provide a high quality and effective social work service that responds to the needs of people with care and support needs, their informal carers, and families in a timely manner and within statutory guidelines. All assessments and support plans will be completed in accordance with the wellbeing principles of the Care Act 2014, reflecting a strengths-based approach.

Experience
Qualified Social Worker with experience in adults services, preferably within hospital discharge or related settings.

Committed to delivering person-centred care and support, with strong communication and organisational skills. Ability to work effectively within a multidisciplinary team and manage caseloads in line with statutory requirements.

Qualifications
Qualified Social Worker registration is essential. Knowledge of the Care Act 2014 and strengths-based practice is required.
