About the Role
We are seeking an Adult Social Worker to join the Integrated Discharge Team within a fast-paced hospital setting. The role involves carrying out Mental Capacity assessments, Safeguarding, Care Act 2014 duties, and Discharge to Assess processes. The team works collaboratively to support patient discharge and community care planning, offering a flexible hybrid working model that balances office and remote work. Parking is available nearby or onsite, and the location is accessible by public transport.

Experience
A minimum of 2 years post-qualification social work experience is required. Experience working with complex care needs, hospital discharge processes, community care, and review is highly desirable.

About you
You will be confident in carrying out adult social care assessments using strength-based approaches to identify creative solutions. You should be adaptable to a fast-paced hospital environment and able to work effectively both independently and as part of a multidisciplinary team.

Qualifications
A Social Work Degree, DipSW, or CQSW qualification is essential, along with at least 1 year post-ASYE (Assessed and Supported Year in Employment) experience.
