About the Role
We are seeking an Adult Social Worker to join the Hospital Discharge Team. This fast-paced team supports hospital patients by creating effective care plans to facilitate timely and safe discharge. You will be based on site carrying out assessments for individuals, working closely with multidisciplinary teams to ensure smooth transitions from hospital to community care. The role involves using strength-based approaches to creatively identify solutions tailored to each person's needs.

Experience
Minimum of 1 year post ASYE experience is required. Experience working within safeguarding hubs, hospital settings, community care, and with older people and those with physical disabilities is essential.

About you
You will be skilled in carrying out adult social care assessments and applying strength-based approaches. You are adaptable, compassionate, and able to work effectively in a fast-paced environment.

Qualifications
Social Work Degree, DipSW, or CQSW qualification is required.
