About the Role
We are seeking an experienced Adult Social Worker to join the Integrated Discharge Team within a fast-paced hospital setting. The role involves carrying out Mental Capacity assessments, Safeguarding, Care Act 2014 duties, and Discharge to Assess processes. The team works closely within the hospital environment to support timely and effective discharge planning. This position offers flexibility with hybrid working options, allowing you to work both from home and onsite. Parking is available nearby or onsite, and the location is accessible by public transport.

Experience
A minimum of 2 years post-qualification social work experience is required, including at least 1 year post ASYE. Experience working with complex care needs, hospital discharge processes, community care, and review is highly desirable.

About you
You will be skilled in conducting adult social care assessments using strength-based approaches to identify creative solutions. You should be confident working in a fast-paced hospital environment and able to manage multiple priorities effectively.

Qualifications
A Social Work Degree, DipSW, or CQSW qualification is essential.
