About the Role
We are currently seeking an experienced Adult's Social Worker to join a proactive Reviews Team. The team works with vulnerable adults in the community to review existing care plans, packages, and placements to ensure appropriate levels of support and funding. Responsibilities include assessing service users under the Care Act 2014 and presenting cases to the commissioning panel when necessary. The team is ambitious and committed to reviewing and adjusting all packages within reasonable timescales. This role offers a rare locum opportunity within a specialist team and includes a hybrid working scheme with easy access by car or public transport.

Experience
A minimum of two years post-qualification experience working in Adult's community-based teams is essential. Recent and comprehensive experience completing learning disability placement reviews is required.

About you
You will be proactive, detail-oriented, and committed to ensuring the best outcomes for vulnerable adults. Experience working within a specialist reviews team or similar environment is highly desirable.

Qualifications
A Social Work qualification is required (Degree/DipSW/CQSW) with at least two years post-qualification experience.
