About the Role
We are seeking an experienced Adult Social Worker to join the Integrated Discharge Team within a fast-paced hospital setting. The role involves carrying out Mental Capacity assessments, Safeguarding, Care Act 2014 duties, and Discharge to Assess processes. The team provides a supportive environment with flexibility to work both remotely and onsite. You will be responsible for conducting necessary adult social care assessments and applying strength-based approaches to identify creative solutions for complex care needs and hospital discharge planning.

Experience
A minimum of 2 years post-qualification social work experience is required, including at least 1 year post ASYE. Experience working with complex care cases, hospital discharge, community care, and reviews is highly desirable.

About you
You will hold a Social Work Degree, DipSW, or CQSW qualification and demonstrate the ability to carry out thorough assessments using strength-based approaches. You should be confident working in a fast-paced hospital environment and able to manage a variety of adult social care responsibilities with professionalism and creativity.

Qualifications
A recognized social work qualification is essential, with a minimum of 2 years post-qualification experience.
