About the Role
We are seeking an Adult Social Worker to join the Reablement Team, a fast-paced environment working closely with hospital services. This team supports individuals being admitted back into the community setting by carrying out discharge to assess, continuing healthcare assessments, and other relevant legislative assessments. The role offers an introduction training program along with ongoing continuous support throughout the post. The team operates on a hybrid working model, balancing office and remote work. Parking is available nearby or onsite, and accessible public transport options are close by. The team is supported by an award-winning compliance team and a dedicated senior recruitment consultant with extensive sector experience.

Experience
A minimum of 1 year post ASYE experience is required. Experience working with hospital, community, and/or safeguarding teams is desirable.

About you
You will hold a Social Work Degree, DipSW, or CQSW and have at least one year of post-ASYE experience. You should be motivated to work in a fast-paced environment and able to collaborate effectively with multidisciplinary teams.

Qualifications
A recognized Social Work qualification is essential for this role.
