About the Role
We are looking for an Adult's Social Worker to join a Community Mental Health Team. This role requires a Social Work Qualification with a minimum of 2 years post-qualified experience. The team operates in a multidisciplinary environment to provide appropriate support to individuals with mental health concerns and their carers. Key responsibilities include undertaking placement reviews, facilitating hospital discharges, and completing safeguarding processes under the Care Act 2014. The team is dedicated to working with each service user as an individual and tailoring their care plans as necessary.

Experience
