About the Role
We are looking for an experienced Adult's Social Worker to join a Joint Emergency Team. The team is responsible for responding to emergency enquiries out of working hours, with a significant aim of ensuring people are supported within their own homes to avoid the need for hospitalization. Recently, the team has been tasked with completing Continuing Health Care assessments for individuals without an allocated social care member of staff. The team consists of expert practitioners in Nursing, Occupational Therapy, Physiotherapy, General Practice, and Social Work.

Experience
Experience working within an Emergency Duty Team and CHC assessment experience is essential for this role.

About you
You must have a degree in Social Work (Degree/DipSW/CQSW) with a minimum of 2 years post-qualified experience. A valid UK driving licence is preferred but not essential.

Qualifications
Social Work Qualification with at least 2 years of post-qualified experience.
