About the Role
We are looking for an experienced Adult's Social Worker to join a Community Neuro Rehabilitation Team. The team provides crucial adult social care interventions, including Care Act Assessments, MCA, Safeguarding advice, information and referrals, with a focus on achieving both individual patient goals and service targets for efficient patient flow and hospital admission avoidance. Additionally, the team supports the NETT team to facilitate a smooth transition for complex neurological patients from hospital/rehab units back into the community.

Experience
A minimum of 2 years post-qualified experience is required, preferably within a Hospital and Mental Health Team.

About you
A degree in Social Work (Degree/DipSW/CQSW) is essential. A valid UK driving licence and vehicle are preferred but not essential.

Qualifications
Social Work Qualification is required.
