About the Role
We are seeking an experienced and motivated Senior Practitioner to join our Adults Social Care team. In this pivotal role, you will manage a complex caseload while providing expert guidance, professional support and case management oversight to social care colleagues. You will undertake assessments for adults with complex and high-risk needs, lead safeguarding enquiries, chair safeguarding meetings, and support the Team Manager in ensuring high-quality service delivery. The post also includes deputising for the Team Manager when required and contributing to the operational leadership of the team.

Working within a multidisciplinary environment, you will collaborate with colleagues across adult services, health partners, housing, public health, voluntary sector organisations and other agencies to deliver person-centred and joined-up services. The team is passionate about safeguarding adults at risk and embraces a strengths-based approach that enables individuals and their carers to make informed choices and maintain independence wherever possible.

Requirements

Experience
Substantial post-qualification experience within a statutory health or social care setting, ideally within a multidisciplinary environment. Proven experience managing and developing staff, identifying and managing risk, and working in partnership with a range of agencies to deliver effective interventions.

About You
Passionate about safeguarding and achieving positive outcomes for adults and their carers. Able to communicate clearly and confidently, produce high-quality reports, manage competing priorities and use performance information to drive service improvements.

Qualifications
Registration with the Health and Care Professions Council (HCPC) or the relevant professional body is essential. A detailed understanding of the legislative and policy framework underpinning adult social care and safeguarding is required.
