About the Role
The WEoLCCS provides a dedicated, specialised care coordination service and central point of contact for patients likely to be in their last year of life and those who care for them (formally and informally). The main responsibilities of the Lead Nurse include acting as a community nursing end of life champion, directly managing and leading the Services Health & Personal Care Delivery Team, providing clinical support to team members, and working closely with the care delivery team, Continuing Health Care, and care agencies to ensure care is tailored to meet the unique and changing needs of patients and their families. Additionally, the Lead Nurse will build and maintain professional relationships both internally and externally, promoting Trinity's services, and will work flexibly within the Clinical Hub Services team as needed.

Experience
Experience in community nursing and end-of-life care is preferred.

Qualifications
A relevant nursing qualification and registration with the Nursing and Midwifery Council (NMC) are required.
