The post holder will be responsible for a caseload of patients and will act as Key Worker. They will be responsible for proactive management of these patients, providing a link while patients are at home and ensuring necessary onward referrals are in place. The post holder will work with colleagues and the Lead Nurse for Cancer to deliver a supportive nursing service for patients and their families undergoing treatment and investigations for cancer conditions at Guys and St Thomas' NHS Trust
