About the Role
As a Community Children's Nurse, you will provide case management for a designated caseload of children and young people with complex needs, utilizing and promoting evidence-based care. You will need excellent interpersonal skills to manage complex discussions with various agencies and individuals, including care providers, patients, and their families. Working in partnership with parents and multi-disciplinary agencies, you will deliver skilled care to children and young people within the community, supporting safe discharge and avoiding unnecessary hospital admissions. The role requires a highly organized individual with a flexible approach to work, capable of handling challenging situations autonomously while demonstrating excellent communication skills.

Experience
Experience in case management of children and young people with complex needs is essential.

Qualifications
