Post holders will need to: Have an excellent knowledge of Mental Health conditions, therapies and current mental health legislation. Be responsible for the full mental health assessment of care needs of patients presenting with a wide variety of clinical conditions. Following this, the post-holder will be responsible for development, implementation and evaluation of programmes of care without supervision, unless otherwise indicated. Demonstrate the ability to support and apply a holistic, person-centred, approach to assessing the health and social care needs of people experiencing severe and enduring mental illness, who have been referred to the Hub. Have a flexible, multi-disciplinary approach and have a solid understanding of risk assessment, care planning and the recovery model. You will also need to be able to offer support and advice to less experienced colleagues.
