The Community Psychiatric Nurse (CPN) role at South London and Maudsley NHS Foundation Trust offers a unique opportunity to provide vital healthcare services within the community. The role involves conducting robust assessments of patients referred for mental health difficulties, identifying appropriate care pathways, and ensuring the least restrictive options to meet patient needs. You will also be responsible for signposting to appropriate care providers, raising concerns to senior staff, completing documentation, and planning care for patients. Collaboration with a Multi-Disciplinary Team is essential for discussing care pathways and ensuring comprehensive patient support.
Experience
A minimum of 6 months paid experience as a Community Psychiatric Nurse in a UK hospital environment within the last 3 years.
About you
You must comply with NMC Professional Registration and keep up to date with current nursing practices. A full UK driving licence and own vehicle are required. Depending on the Trust, you may require variable additional certification and skills.
