About the Role
The Community Recovery Service Assessor role is an exciting opportunity to support vulnerable residents in Hounslow by promoting independence through rehabilitation and reablement. You will work within an integrated health and social care team alongside Hounslow and Richmond Community Healthcare NHS Trust, providing assessments, support planning, and monitoring to help residents recover following injury, illness or changes in life circumstances. The role involves close partnership working, performance management, mentoring and supervision of Rehab Assistants. You will contribute to service reviews, quality monitoring visits and help identify when additional support or equipment is required. Flexibility is essential to meet the needs of residents, carers and staff across a wide range of hours. This role offers the chance to develop creative strategies and improve ways of working within a dynamic and supportive team environment.

Experience
- Experience in carrying out assessments and developing support plans is essential.
- Ability to use business systems, information technology, spreadsheets and write reports.
- Experience working in health, social care or rehabilitation settings is desirable.

Qualifications
